You remember 2020. The country was scared. And when people get scared, they look to the people in white coats. Gallup’s annual survey found that 51% of Americans had a “great deal” or “quite a lot” of confidence in the medical system that year—the highest reading in the poll’s 30-year history. A 15-point jump in a single year. That almost never happens.
Then came the arguments. Mask mandates. Vaccine requirements. Lab leak theories brushed aside, then taken seriously, then brushed aside again. Lockdown policies that crushed small businesses while big-box stores stayed open. People watched, and they made judgments. By 2022, confidence had dropped to 38%. By last summer, Gallup’s June 2025 survey put it at 32%—lower than any reading in the poll’s 30-year history. A 19-point collapse in five years.
And it isn’t just the “system.” Trust in individual doctors fell 14 points between 2021 and 2024, the steepest drop Gallup has recorded for any profession. Nurses still top the trust list at 75%, but even their numbers have softened. A record 23% of Americans now say the healthcare system is “in a state of crisis.” Not “has problems.” Crisis.
Now here’s what most people get wrong. They think this is only about COVID politics. It’s bigger than that. Gallup’s March 2026 poll found that 61% of Americans now call healthcare affordability their top domestic concern—10 points ahead of the economy. One in nine adults says they can’t pay for needed care or medicine, a new high. A third of the country has cut back on daily expenses just to cover medical bills. The pandemic didn’t create the distrust. It just pulled the curtain back on a system that was already too expensive and too bureaucratic for ordinary people.
What’s left is an American public that used to give medicine the benefit of the doubt—and doesn’t anymore. That changes how people make decisions about their own health, what treatments they accept, and who they vote for. It’s one of the biggest shifts in public opinion this century, and almost nobody is talking about it.
